Please be on the lookout for stolen Rx pads from Blaine Lisner, M.D. in Louisville, Kentucky. The office is located at 3934 Dixie Hwy. #345. They reported numerous prescription blanks were taken from their facility. If you receive a suspicious written Rx, please try and get as much information as possible, save any recorded messages and notify your local police department. Also, please note any vehicle or suspect descriptions if someone arrives to pick up a prescription written by Dr. Lisner. Thank you!
Detectives can be reached regarding this or any other suspected prescription related offense at 502-574-6272 or at any of the numbers attached. Remember, in an emergency, call 9-1-1.

They recently had a burglary and reported missing controlled substance prescription blanks from their facility. Some of the prescribers listed on the prescription blanks include: Jonathan B. Hanson, MD, John P. Hossler, MD, Alan P. Culbreth, MD, Phillip A Ross, MD, Mark D. Totten, MD, Larry V. Williams, MD, Vikki Conners, FNP-C, Scott H. Murphy, MD, Roy W. Miner, MD, and Steven D. Kentucky Emergency Preparedness Communication
Kentucky Medicaid has activated emergency procedures for members affected by the flooding in Eastern Kentucky on July 28th, 2022. These procedures will be terminated as of August 31, 2022.
The member must identify themselves as having lost their medication due to the recent flooding. The pharmacist can then utilize the Submission Clarification Code (420-DK) =13 indicating Payer-Recognized Emergency/Disaster Assistance Request. By utilizing this Submission Clarification Code, the pharmacist is indicating that the override is needed due to the emergency that is recognized by the payer.

2014 Plan Design Changes
On January 1, 2014, Kentucky Medicaid will implement new copayments and cost sharing policies. The new copay structure will be as follows:
- $1 for all generics
- $4 for preferred brands
- $8 for non preferred brands
Exceptions to the above copay requirements will be:
- The following drug classes:
- Family planning, no copays
- Tobacco Cessation, no copays
- 2nd Generation Antipsychotics and Injectable Antipsychotics, $1 copay
- Anticonvulsants, non-preferred brands, $4 copay
- Oral Oncology, non-preferred brands, $4 copay
- Diabetic Supplies
- Meters, no copays
- Test strips, control solutions, insulin needles, lancets, etc., $4 copay with no more than one copay per calendar day being charged
- The following eligibility groups are excluded from the generic ($1) and preferred brand ($4) copay; however, they ARE subject to the non preferred brand ($8) copay:
- Children defined as age <19 years, except for those children eligible under the KCHIP 3 program:
- Children enrolled in the KCHIP 3 kids plan will be subject to the generic ($1) and preferred brand ($4) copay, as well as the non preferred brand ($8) copay.
- Pregnant women, as identified by NCPDP Field # 335-2C = "2" (pregnant)
- LTC members, as identified by patient residence field (NCPDP Field # 384-4X) = "2", "3", "4", "5", "6", or "9". In the case of a partial fill, this copay will be due in full at the time of the first fill.
- Children defined as age <19 years, except for those children eligible under the KCHIP 3 program:
- Copayments will be accumulated so that they do not exceed 5% of the family income on a quarterly basis.

National Prescription Drug Toss - Saturday, October 26, 2013
The Drug Enforcement Administration (DEA) has designated Saturday, October 26, 2013, from 10:00 a.m. to 2:00 p.m. as a drug take-back day.
The National Prescription Drug Take-Back Day aims to provide a safe, convenient, and responsible means of disposing of unnecessary prescription drugs, while also educating the general public about the potential for medication abuse.
According to a recent report by the Trust for America's Health, Kentucky has the third-highest rate of fatal overdoses. The majority of these deaths are from prescription medications.
There are currently more than 70 prescription drug collection sites in Kentucky that will be open on Saturday, October 26th, from 10:00 a.m. to 2:00 p.m. for drug tossing. Please refer to the following link for additional information or to locate a collection site near you.

Benzodiazepine Duration Edit Override
Effective August 15, 2013, the prescriber must call 800-477-3071 for all requests for override of the benzodiazepine duration edit.
Attention Kentucky Medicaid Members: New Warnings about the use of Acetamenophen (Tylenol)
On August 1, 2013, the Food and Drug Administration issued a warning that acetaminophen (Tylenol) products may cause serious skin reactions. Prescription medications containing acetaminophen (Tylenol), over-the-counter medications containing acetaminophen (Tylenol), products containing only acetaminophen (Tylenol), and products containing acetaminophen (Tylenol) in combination with other medications may cause serious skin reactions. These skin reactions may cause reddening of the skin, rash, blisters, and detachment of the upper surface of the skin. These reactions could be life threatening. If you have a skin reaction and you are taking acetaminophen (Tylenol), stop taking the medicine immediately and call your doctor.
Additionally, to help prevent the potential for serious liver disease, members are encouraged to keep their daily dose of acetaminophen (Tylenol) to no more than 3,000 mg per day.

These prescriptions are usually picked up the following day in the drive thru. Implementation Date for Medicaid Managed Care Extended
On September 19, 2011, the Cabinet for Health and Family Services (CHFS) Secretary Janie Miller announced that the implementation date for Medicaid Managed Care, targeted for October 1, 2011, will be extended one month, an option provided in the contracts with managed care organizations. The Cabinet has granted the extension in response to a request from the Kentucky Hospital Association (KHA).

Currently someone is calling in several fraudulent prescriptions using Dr. Noel's information. These are typically being called in after 4:00 pm or anytime during the weekend, when Dr. Noel is out of the office. The caller is identifying themselves as "Theresa." The prescriptions are for Lortab.

Rx Alert! ALL PHARMACIES IN THE LOUISVILLE METRO AREA
A white male with dark blonde hair and blue eyes, estimated to be in his early 30’s is the listed subject in possession of stolen Norton Leatherman Spine Institute prescription blanks. The subject is having the prescription blanks filled for Lortab 10/500 #180 and #240 at several pharmacies throughout Metro Louisville. Subject is also phoning in prescriptions claiming to be Dr. John Johnson of the Leatherman Spine Center. Again, all phoned in prescriptions are well into the #180 and #240 dosage unit range. The detective has identified seven (7) individual names being used to obtain this medication. This individual has used his ID and signature to obtain the prescriptions. He has also used Dr. Robert Couch and Dr. David Roubin.

UPDATE: Zyvox® Prior Authorization Criteria
The Department for Medicaid Services has made a change in policy related to the use of Zyvox®. The Department will closely follow the new recommendations from the Pharmacy and Therapeutics Advisory Committee in conjunction with Infectious Disease experts in the state for the appropriate utilization of Zyvox®. As a result, a drug specific prior authorization fax form has been developed.

UPDATE: 2009-2010 RSV Season
The Department for Medicaid Services has made a change in policy related to the use of Synagis®. The Department will closely follow the new recommendations from the American Academy of Pediatrics for the appropriate utilization of Synagis® for the prevention of respiratory syncytial virus (RSV). As a result, the 2009 RSV season will begin November 1, 2009 and continue through March 31, 2010.

Please be on the lookout for stolen Rx pads from Blaine Lisner, M.D. in Louisville, Kentucky. The office is located at 3934 Dixie Hwy. #345. They reported numerous prescription blanks were taken from their facility. If you receive a suspicious written Rx, please try and get as much information as possible, save any recorded messages and notify your local police department. Also, please note any vehicle or suspect descriptions if someone arrives to pick up a prescription written by Dr. Lisner. Thank you!

Please be on the lookout for stolen Rx pads from Kings Daughters Hospital (Hilltop Clinic, 445 Clifty Drive) in Madison, Indiana. They recently had a burglary and reported missing controlled substance prescription blanks from their facility. Some of the prescribers listed on the prescription blanks include: Jonathan B. Hanson, MD, John P. Hossler, MD, Alan P. Culbreth, MD, Phillip A Ross, MD, Mark D. Totten, MD, Larry V. Williams, MD, Vikki Conners, FNP-C, Scott H. Murphy, MD, Roy W. Miner, MD, and Steven D. Adams, MD. If you receive a suspicious written Rx, please try and get as much information as possible, save any recorded messages and notify your local police department. Currently there is an unknown female phoning in prescriptions and identifying herself as "Dawn" or "Donna" from the Leatherman Spine Center. She is ordering prescriptions using the names & DEA #s of Dr. Rolando Puno or Dr. Steven Glassman for Lortab 10 #60 or Lorcet 10 #40 and a variety of fictitious patient names. These prescriptions are usually picked up the following day in the drive thru. If you receive a similar call, or any suspicious call, please try and get as much information as possible, save any recorded messages and notify detectives at 574-6272. Also, please note any vehicle or suspect descriptions if someone arrives to pick up any of these prescriptions.

Rx Alert! Stolen Prescription Pads
Alert 1: Recently the office of Doctor Richard Goldstein in Charlestown, IN was burglarized. During the burglary several controlled substance prescription blanks were stolen. These will probably be used both in Indiana and Kentucky. If you receive any of these prescriptions, please call the LMPD prescription Drug Diversion Unit at 502-574-6272.
(In Indiana you can call the Sellersburg ISP Post at 812-246-5424. If you have information regarding this burglary, please call the Sellersburg Post and ask to speak with Trooper Tracy Spencer.)
Alert 2: Be on the lookout for stolen or suspicious prescriptions from Doctor Jagdish Kothari, Dr. Renu Mehta and Dr. Mini Aggarwal. A pad of prescription blanks is missing. Please call the Prescription Drug Diversion Unit at 574-6272 if you receive one of these prescriptions.
Rx Alert! Louisville Area Pharmacies
Prescriptions for Dr. Noel, 3612 Lexington Rd, Louisville, KY 40207
Currently someone is calling in several fraudulent prescriptions using Dr. Noel's information. These are typically being called in after 4:00 pm or anytime during the weekend, when Dr. Noel is out of the office. The caller is identifying themselves as "Theresa." The prescriptions are for Lortab.
If you receive any of these phoned prescriptions, please contact the LMPD Prescription Drug Diversion Unit at (502) 574-6272. If it is an emergency, call 9-1-1
Rx Alert! ALL PHARMACIES IN THE LOUISVILLE METRO AREA
A white male with dark blonde hair and blue eyes, estimated to be in his early 30’s is the listed subject in possession of stolen Norton Leatherman Spine Institute prescription blanks. The subject is having the prescription blanks filled for Lortab 10/500 #180 and #240 at several pharmacies throughout Metro Louisville. Subject is also phoning in prescriptions claiming to be Dr. John Johnson of the Leatherman Spine Center. Again, all phoned in prescriptions are well into the #180 and #240 dosage unit range. The detective has identified seven (7) individual names being used to obtain this medication. This individual has used his ID and signature to obtain the prescriptions. He has also used Dr. Robert Couch and Dr. David Roubin.
Please notify the LMPD Prescription Drug Diversion Unit at (502) 574-6272 if you encounter this individual. Dial 9-1-1 in case of emergency.
